About care and support

Do you want to continue to live at home?

We know it’s important to keep your independence and stay living in your own home for as long as possible.

We’ve put together some great advice, along with details of many services and support groups that can help you with this.

If you are finding it difficult to remain independent on your own, you can ask us to help you identify the care and support you need so that you can continue to live life the way you want to.

We can also help you to access this care and support, both in the community, and within our specialist services.

Who this applies to

We can offer support to you if you:

  • are aged 18 or over
  • live independently at home or in a care home
  • are a resident of East Riding of Yorkshire

and have a care and support need because you:

  • are older or frail

or you have:

  • a physical and/or learning disability
  • mental health issues, memory problems or dementia
  • problems with drugs and/or alcohol.

    We know it’s not always easy to take on board information and say what you want to say when decisions are being made about your life. An advocate can help clarify points for you and get across your views and feelings to make sure you get the best outcome for you.
