About needs assessments

Do you want to continue to live at home?

We know it’s important to keep your independence and stay living in your own home for as long as possible.

We’ve put together some great advice, along with details of many services and support groups that can help you with this.

If you're finding it difficult to remain independent and cope day to day, we can help you access adult social care services.

After you've contacted us to see how we can help, we’ll complete a needs assessment to find out if you qualify to receive any services from us.

Who this applies to

We can offer you support to you if you:

  • are aged 18 or over
  • live independently at home or in a care home
  • are a resident of the East Riding of Yorkshire

AND have a care and support need because you:

  • are older or frail

or you have:

  • a physical and/or learning disability
  • mental health issues, memory problems or dementia
  • problems with drugs and/or alcohol.

How it works

There's a process that we must follow to decide what care and support you need and how it should be provided.

  • Needs assessment

    You'll have a needs assessment so we can identify what:

    • your care needs are (often referred to as 'eligible needs')
    • kind of care and support you need.
  • Care and support plan

    Your needs assessment will give us an idea of the care and support you need, and we’ll be able to estimate how much it will cost.

    The cost of your care and support is known as your ‘personal budget.’ You will not have your personal budget amount confirmed until your financial assessment has been carried out. You can, however, use the amount we estimate to put together a care and support plan - we’ll help you to do this.

    The plan will show us how you intend to use the money to meet your care needs, and will be used alongside your financial assessment to confirm your final personal budget amount.

  • Financial assessment

    Once we have an idea of how much your personal budget will be, we can also carry out a financial assessment.

    This will look at your financial situation and work out if you can afford to pay anything towards your personal budget. Most people must contribute something towards their care, and some are required to pay the full amount.

  • Your final personal budget amount

    We'll look at your care and support plan to check we are happy with how you have planned to spend your personal budget. We’ll then approve it and confirm the final amount for your personal budget in writing.
