Reviewing your care and support plan

Do you want to continue living at home?

We know it’s important to keep your independence and stay living in your own home for as long as possible.

We’ve put together some great advice, along with details of many services and support groups that can help you with this.

We understand that your care and support needs may change over time, so we’ll invite you to regular reviews.

If your needs change before your yearly review, or you feel that your care and support plan isn’t meeting your needs, you can request a review at any time. Just speak to your adult social care worker.

6 week review

6 weeks after your plan has been agreed and your care and support has been put in place, we’ll contact you to arrange an appointment for an initial review.

This review is to make sure that all your needs have been met and everything in the plan is going well.

1 year review

We'll contact you around a year later to check your care and support plan is still meeting your needs.

At this appointment it is important to let us know if anything has changed so we can check if we need to adapt the care and support you receive.

You will have a review each year while we are providing your care.
